Qualifications: Bachelors’s Degree from an accredited college or university in Social Work, Psychology or Human Services with at least 1-2 years of experience. Must possess a valid driver’s license in good standing and have access to a vehicle.

Responsibilities: Providing assertive outreach and intensive case management to homeless individuals with mental illness who reside in Safe Haven. Assessing consumer needs and developing a housing plan for each shelter guest to transition to permanent housing. Assist individuals with utilizing all available community supports that best meet their needs including mental health treatment, medical/dental, financial, employment and education as appropriate. Provide 24/7 support to those served by the program through on call rotation. Assist consumers with applying for affordable housing as well as regularly searching the housing market for affordable units. Documenting all contacts and maintaining timely and accurate program records as required. Attend regular supervision as well as staff meetings and required training. Must meet all expected productivity levels in regards to the direct service. Perform other duties as assigned by the Director.

Skills: Must have good oral and written communication skills and be detail oriented and able to efficiently manage and prioritize numerous tasks and complete work within deadlines. Strong knowledge of local community services is needed. Ability to assess for psychiatric crisis and intervene appropriately. Knowledge of local community services. Knowledge of computer applications. Able to work within a busy environment and remain focused and prioritize service delivery to consumers in need.  Interacts with a diverse population in a supportive and respectful manner.
